Facility Comments: Sentara Home Care Services provides home health, pharmacy/infusion therapy, and Philips Lifeline Personal Emergency Response System. We work closely with physicians to coordinate the multiple therapies and services each patient may require - all in the comfort of home. Services provided include skilled nursing (from wound care to chronic disease management to rehabilitative therapies), as well as Telehealth remote monitoring to measure patients' vital signs for more frequent observation. Home Pharmacy/Infusion for acute and chronic illnesses, including specialty medication therapies and ambulatory infusion pumps, is provided by our expert clinical staff and pharmacists who work closely with the patient's physician to support every aspect of home infusion needs.
